SBTC Membership Application 2021

Welcome to the SBTC Registration Form.  Please read these instructions before beginning to register.  Note: All Programs are currently suspended due to COVID-19 restrictions; they may be offered at a later time, if this situation changes.

  • Only the Primary Contact information and COVID/Indemnity waivers (red *) must be filled in. All other fields are optional - please use common sense to decide which fields are appropriate for your registration.
  • If you choose Family membership, please remember to choose "Family" as the Membership Type for all members.
  • If you are registering Junior Member(s) only, please complete the Primary Contact area and the appropriate Junior Member area(s). (Note: Junior Member must be 5 years old as of December 31, 2020)
  • Please fill in Date of Birth and Gender info for all applicants. For all DATES: please select a drop-down year, then a drop-down month and then click on the day on the calendar.
  • The provides a HELP note relating to the field and how it should be filled in. Please mouse over and read each HELP note before filling in the item, especially the Indemnity Waiver.
  • Before proceeding to payment, check your Total Cost carefully, to ensure it is the correct total that you expect.  You can use the Preview button at the bottom of the form to review your form submission before submitting it for payment.  (Note: The "Reset" button will clear your form in case you need to start all over again.)
  • Secure payment is via PayPal. You can use a PayPal account or pay by Credit or Debit card.
